Please join the Central Pennsylvania Chapter for our October Program, "Population Health Management" on Wednesday, October 25, 2017 from 8:15 to 10:00 am at the Highmark Blue Shield Office, 1800 Center Street, Camp Hill, PA.

​Our speaker, Dr. Mark Jacobson, DO, will discuss trends in chronic disease status and overall health trends in populations attributable to the Central Pennsylvania market and the programs that are available to address these trends via wellness and disease management.

Mark Jacobson, DO
Medical Director – Clinical Client Relations
Highmark Blue Shield

Dr. Jacobson works collaboratively with Sales and other internal teams to positively influence client acquisition and retention by providing clinical consulting strategies. He guides the progression of these strategies, presents the rationale and results to clients, and develops strong relationships with clients by offering his clinical observations and insights both at and between formal client meetings. Prior to joining Highmark, he served as VP of Medical Affairs and as a consultant of WellSpan Population Health Analytics. Board certified in internal medicine, he has held positions at Keystone Health Plan Central, Holy Spirit Health System and other central Pa. region health plans and systems. Dr. Jacobson earned his medical degree at the Philadelphia College of Osteopathic Medicine. He has served on the Board of Directors of the Holy Spirit Health System, the American Cancer Society-Lebanon Division and the American Heart Association-Lancaster Division. He is a member of the Pennsylvania Osteopathic Medical Association, and currently holds a teaching appointment as Adjunct Associate Professor of Public Health Services at the College of Medicine of Penn State University.
